Clinical Enrollment
A brief eligibility check confirms the patient qualifies under program criteria. Enrollment takes under 10 minutes and can be completed by a care coordinator, social worker, or CHW.
Device Delivery & Setup
A trained CarePhones community health worker delivers the device in person — at home, at the clinic, or at a community site. They configure telehealth apps, walk through usage, and answer questions in the patient's preferred language.
Ongoing Patient Support
Patients have access to a dedicated CarePhones support line for 12 months. Our team handles technical issues, app updates, and connectivity troubleshooting — so your clinical staff don't have to.
Outcomes Reporting
You receive quarterly outcomes reports tied to your program goals — readmission rates, telehealth utilization, care gap closure, and patient satisfaction scores. All data is HIPAA-compliant and de-identified.
SIM-Based. No Wi-Fi Required.
Every CarePhone ships with a pre-activated SIM card on a nationwide network. Patients don't need home broadband, a Wi-Fi password, or a monthly bill. Connectivity is included in the program cost — billed to the health system, not the patient.
